HEALTH GUARANTEE STATEMENT
The health of this dog is excellent to the best of our knowledge at the time of sale. The
required vaccinations and worming are current as indicated on the Health Record.
This dog is guaranteed to be free of any non-treatable, life threatening genetic defects for
a period of ONE YEAR from the purchase date. Any genetic defect must be confirmed by a
competent, licensed veterinarian. A complete report must be sent to Dazzling Puppies within
48 hours after the determination. Dazzling Puppies is not responsible for any vet or shipping
costs incurred by the buyer.
Dazzling Puppies will not replace dog if there are any signs of neglect or abuse. In the
event of death of dog; cause must be determined by autopsy at the expense of buyer. If cause
of death is found to be genetic Dazzling Puppies will replace dog if death is within ONE YEAR of
purchase date.
Dog must be examined by a veterinarian within 7 days of purchase date for this guarantee
to be valid. Buyer must contact Dazzling Puppies and give vet check results within the week.
Dog is guaranteed against fatal viruses including: distemper, parvo, and corona for ONE
WEEK. This does not include hypoglycemia giardia, coccidia, kennel cough and any internal or
external parasites.
This guarantee is valid for a replacement dog of equal value only. UNDER NO
CIRCUMSTANCES will any refunds be given except at Dazzling Puppies discretion.
In case a defect is found dog must be returned to Dazzling Puppies to qualify for a
replacement dog. Buyer must feed quality dog food (such as Eukenuba or Royal Canin) and
keep dog current on worming and vaccines.
Buyer understands that Dazzling Puppies cannot guarantee the temperament and
behavior which this dog will hereafter exhibit, that the temperament and behavior this dog
will hereafter exhibit will be a function of, among other things, the treatment, training and
discipline the dog hereafter receives and that Dazzling Puppies will bear no responsibility for
or involvement with this dog in any such matters after the Buyer’s purchase. Accordingly, in
consideration for Dazzling Puppies agreement to sell this dog to the Buyer, the Buyer hereby
releases and forever discharges Dazzling Puppies from any and all claims or liability for any
damage, injury or death caused by this dog to the Buyer or any minor child of the Buyer. In
addition, the Buyer agrees to indemnify, defend and hold the Seller harmless from any and all
claims or liability for any damage, injury or death caused by this dog to the Buyer, any minor
child of the Buyer or anyone other than the Buyer or a minor child of the Buyer.
Buyer agrees that seller will not be liable for anything whatsoever other than the terms
laid out in this contract. Buyer understands that seller cannot guarantee size or breeding
ability. In the event of dispute Indiana will have jurisdiction over the matter in Elkhart County.
The Buyer shall reimburse Dazzling Puppies for all costs and expenses, including attorney’s
fees, incurred by Dazzling Puppies in enforcing the terms of this Statement.
If any terms of this agreement are ignored or violated this contract will be null and void.
